All Columbia Public Library programs are free and available to the public!

However, library events that involve physical or strenuous activities, food, or potentially harmful equipment or machines will require all participants to sign one or more of the following liability forms.

All forms are available at the circulation desk. Also, at your convenience, feel free to download a (PDF) form, print, fill out, and then return to library’s front desk before your upcoming event.

• Food Allergy Waiver
• Photo Release Form for Adults
• Photo Release Form for Minors

**All individuals only need to have one of each form signed a year.
